Job Overview

Key Responsibilities
Deliver comprehensive nursing care, including management of chronic conditions, wound care, immunizations, and health screenings.
Conduct patient assessments, create care plans, and ensure delivery of patient-centered care.
Work collaboratively with GPs, healthcare assistants, and other multidisciplinary team members to ensure seamless patient care.

Maintain accurate patient records in line with relevant guidelines and protocols.
Qualifications And Experience
Registered Nurse (RGN) with a current NMC pin.
Experience in a primary care or community healthcare setting is highly desirable.
Skilled in managing chronic diseases such as diabetes, asthma, and COPD.
Excellent communication skills and the ability to work effectively within a team.
Proficient in using clinical systems and electronic patient records.
