We’re seeking a dedicated and culturally responsive Case Manager to deliver personalised, strengths‑based mentoring and support to First Nations participants involved in the justice system. This role plays a vital part in supporting individuals through transition stages, helping them reconnect with community, culture and essential services, and build pathways to employment, education and training.
About the role
As a Case Manager, you will work closely with participants both within correctional settings and in the community, providing holistic, trauma‑informed support. You will coordinate services, build trusted relationships, and support positive reintegration outcomes while maintaining high standards of safety, professionalism and cultural integrity.
Key Responsibilities:
Deliver culturally safe, trauma‑informed case management
Develop and maintain Individual Action Plans
Coordinate referrals to health, housing, legal, education and employment services
Maintain accurate case notes and reporting
Support understanding and compliance with parole conditions
Build strong relationships with participants, staff and community stakeholders
Identify and escalate risks in line with safety and wellbeing practices
About You
Experience managing a caseload and prioritising workload
Commitment to culturally responsive, community‑led services
Ability to work effectively in correctional and community settings
Strong communication and relationship‑building skills
Qualifications & Experience
Certificate IV or higher in Community Services, Justice, Social Work or related field
Understanding of First Nations cultures and justice system processes
Proficient with case management systems and digital tools
