Job Overview
We are seeking a compassionate and dedicated Home Support Worker to provide essential care and assistance to individuals in their homes and community settings. Responsibilities
Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
Monitor patient health by taking vital signs, observing symptoms, and reporting changes promptly to healthcare professionals
Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
Administer medications accurately following care plans while adhering to HIPAA regulations for patient privacy and confidentiality
Support clients with medication management, ensuring proper timing and documentation of doses
Maintain a clean, safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces
Provide emotional support and companionship to foster social engagement and reduce feelings of isolation
Skills
Proven experience in home & community care, assisted living, or long-term care environments
Knowledge of patient observation techniques, dementia care, Alzheimer’s care, and memory care strategies
Ability to work effectively with individuals with developmental disabilities or autism spectrum disorders
Certification in first aid and CPR; knowledge of HIPAA compliance for patient privacy protection
Experience operating mobility aids such as Hoyer lifts or resident lifts safely
Familiarity with care plans, medication administration protocols, and vital signs monitoring
Strong communication skills for collaborating with healthcare teams and family members
Physical ability to perform heavy lifting tasks safely when required for resident transfers or equipment handling
