Job Description
As part of our collaborative care team, you’ll work alongside Team Leaders and Rehabilitation Professionals to bring meaningful, rehabilitative activities to residents. Through physical therapy programs, you’ll help enhance their mobility, build strength, improve balance, and increase their independence in daily life. This role is all about creating positive, impactful experiences that support each resident’s journey to their fullest potential.

What You Will Do
Understands, supports and implements the Vision, Mission, Values, and Person Centered Care Philosophy.
Champions a culture of safety. Demonstrates a commitment to the physical, psychological and cultural health and safety of residents, staff, and visitors.
Ensures a safe environment and monitors safe practices and procedures in the facility.
Assesses residents as needed and makes referral to PT/OT as required.
Assists residents with dressing, toileting, lifting, transfers, positioning, feeding and other activities of daily living as required.
Assists individual and groups of residents with established rehabilitative activities such as mobility, strengthening, balance and range of motion exercises, walking programs and activities of daily living as directed.
Demonstrates to residents and assists in the use of exercise equipment such as Pulley system, parallel bars, passive trainers and other therapeutic aids/equipment.
Reports observations of resident’s progress and responses to treatment program. Communicates concerns and / changes to resident’s condition.
Accepts transfers of function from Rehabilitation Professionals (OT/PT).
Porters residents as required.
Sets up and organizes furnishing and equipment for related activities as required; cleans and maintains equipment and reports malfunctioning equipment.
Evaluates treatment plans and discusses with the care team.
Attends and participates in care conference and in-service education as requested.
Maintains inventory of equipment and supplies and reports needs to Rehabilitation Therapy Coordinator.
Participates in the Continuous Quality Improvement program.
Records unusual occurrences and statistical records.
Monitors safety in the residents living environment and staff working environment.
Reports risks, hazards, near misses, unsafe situations, incidents and accidents to CSRN, DOCS or designate.
Performs other related duties as assigned.
Your Qualifications – The Must Have’s
Completion of a Rehabilitation Assistant Program plus one years’ recent related experience or an equivalent combination of education, training, and experience (preferred), Grade 10 education plus graduation from a recognized Resident Care Aide program, or an equivalent combination of education, training, and experience acceptable to Suncreek Village.
Current CPR certification to Level C
Current First Aide certification
Food Safe Level One certification
WHMIS (Workplace Hazardous Materials Information System)
