We are seeking an experienced Remote Care Coordinator to join our team at Doctor Care Anywhere Group. As a key member of our team, you will serve as the first point of contact for patients experiencing mental health concerns, delivering high-quality, patient-centred care through digital platforms.
This critical role involves conducting mental health assessments, providing evidence-based treatment recommendations for primary care mental health conditions. Additionally, you will manage patients in crisis, providing timely and effective support.
Responsibilities include:
Conducting initial remote assessments of patients with mental health concerns
Performing holistic assessments of health, social, and well-being needs
Using evidence-based, patient-centered principles to assess, plan, implement, and evaluate interventions
The ideal candidate will have a strong background in mental health nursing, with experience in conducting remote patient assessments and working in primary care settings.
Qualifications:
NMC Registered Mental Health Nurse
Minimum of two years experience as a Mental Health Nurse within Primary Care or General Practice, practicing at Band 8a or above.
