Referrals & Assessments Receive referrals from GPs, nurses, and other healthcare professionals. Conduct holistic assessments to understand patients needs, strengths, and goals. Prioritise referrals based on urgency, risk, or complexity. Personalised Support Planning Develop personalised care and support plans with individuals.
Motivate and support clients to take active steps toward improving their well-being. Connecting to Community Services Signpost or refer individuals to a wide range of services such as: Housing support Financial/debt advice Mental health services Social clubs and volunteering Physical activity and lifestyle groups Liaise with voluntary, community, faith, and social enterprise (VCFSE) organisations. Follow-up and Monitoring Monitor progress and offer ongoing support where needed. Maintain regular contact and review goals.
Use case management systems to record interactions, outcomes, and feedback. Collaboration and Communication Build strong relationships with healthcare teams, local authorities, and community groups. Attend multidisciplinary team meetings and contribute to patient care plans. Promote awareness and understanding of social prescribing among colleagues.
