Role Summary
We are seeking an enthusiastic and experienced Case Manager to deliver high-quality case management support services to young men, women, and families who are at risk of or experiencing homelessness. This is a unique opportunity to make a positive impact in the lives of those in need.
Ensure ongoing assessments are made of the client's issues, needs, and circumstances from initial intake to end of support period.
Manage situations where a resident is displaying inappropriate and/or anti-social behaviour.
Identify improvements to assist in the development and implementation of processes or programs to address gaps.
Participate in strategic working groups to provide expert input to influence industry or sector development.
