Support Worker

FlexiHome Care provides a wide range of in-home and community care support services to disabled people of all ages. We provide flexible disability support services that are centred on the client's needs and want. We provide our participant services beyond what is expected from us as we want to give them the best life they can achieve within our capability.

Main Duties and Responsibilities

· Provide person-centred care to help customers achieve their goals and aspirations

· Contributing information for the development of Customer support plans

· Providing support to customers with personal care, living skills, transport, social activities, medication and other support as required

· Keeping customer records up to date on a daily basis

· Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders

· With support & guidance from our growing Team, seek out innovative ways to enhance the customer journey

Requirements:

Completed Cert 3 or 4 in disability, Individual support, Aged Care, Nursing
Applicants with Valid License and Access to own vehicle will be prioritized ( License and Comprehensive Insurance)

NDIS Worker Screening Check (or willing to apply)
At least 6 months experience in disability, aged care or nursing sector.
Proof of Vaccination ( COVID 1+)
Working with Children's Check (or willing to apply)

Strong communication, problem solving and conflict resolution skills
Excellent time management skills, as you will be working in a fast paced environment
Excellent time management, multi-tasking and organisation skills.

Excellent attention to detail.
Organised and customer service focused.
