About the role:
We currently have positions available for compassionate, qualified, and reliable Registered Nurse to join our team.
This role goes beyond rostering, you’ll play a vital part in ensuring safe, effective, and high-quality delivery of in-home care services. You will be a key point of contact for clients and families, supporting their wellbeing, guiding staff, and ensuring day-to-day service delivery aligns with care plans and organisational policies.
Key Responsibilities:
Client Care & Case Support
Act as the primary contact for clients and families regarding their day-to-day care needs.
Conduct assessments and contribute to the development, review, and updating of care plans under the direction of the Care Manager, aligned with consumer-directed care, wellness, and reablement principles.
Provide nursing care within scope of practice and support clients in maintaining independence and achieving their care goals.
Workforce Support & Supervision
Support the recruitment, onboarding, and mentoring of care staff.
Ensure staff complete mandatory training (infection control, manual handling, safeguarding).
Provide supervision, feedback, and guidance to care workers to maintain quality service delivery.
Operational Oversight & Scheduling
Manage rosters to support continuity of care and client preferences.
Monitor daily operations, escalating service gaps or risks to the Care Manager.
Contribute to quality monitoring processes in line with organisational procedures.
Compliance & Quality Support
Support the Care Manager to ensure services are delivered in accordance with Aged Care Quality Standards, HCP guidelines, and legislation.
Report incidents promptly to the Care Manager and assist with documentation to meet organisational reporting requirements (including SIRS).
Maintain accurate, timely client records and documentation.
Communication & Advocacy
Build positive relationships with clients, families, carers, and colleagues.
Respond to client queries, feedback, and complaints, escalating complex issues to the Care Manager.
Advocate for client dignity and informed choice while promoting safe and appropriate care.
Financial & Package Guidance
Support clients to understand their package inclusions and exclusions in line with the Inclusions/Exclusions Framework.
Provide practical guidance on using package funds effectively, with oversight from the Care Manager.
Leadership & Continuous Improvement
Lead by example, fostering a culture of professionalism, empathy, and respect.
Participate in service reviews, audits, and quality improvement initiatives led by the Care Manager.
Contribute ideas to improve service delivery and client experience.
About You
Qualifications
Current AHPRA Registration as a Registered Nurse (required).
Relevant tertiary qualification in Aged Care, Nursing, or Management (desirable).
Postgraduate training in dementia care, geriatric nursing, or palliative care (desirable).
Experience
Strong knowledge of the aged care sector, in-home care, CQI, and the Community Care Standards (essential).
Previous experience in aged care, community health, or a related field (desirable).
Skills & Attributes
Strong communication, advocacy, and relationship-building skills.
Ability to assess, plan, implement, and evaluate in-home care.
Proficient in care management systems and digital tools.
Organised, detail-oriented, and committed to compliance.
Collaborative leadership style with the ability to mentor and support staff.
Knowledge of aged care legislation, Quality Standards, the Charter of Aged Care Rights, and the Inclusions/Exclusions Framework.
Requirements
Current AHPRA registration.
Current Australian Federal Police clearance.
Valid driver’s licence and a reliable, fully insured vehicle.
Participation in mandatory and ongoing professional development.
Working Conditions
Community-based role, primarily working in clients’ homes.
Travel required between client locations.
Flexible shifts depending on client needs.
