Job Description
Position: Triage Nurse
Location: Remote after ~3 weeks
Training Location: Pearland, TX 77584
Hours: Monday- Friday: 8am-5pm CST
Duration: Contract with potential for hire
Start Date: ASAP
Position Responsibilities:
• Answer and triage patient phone calls and portal messages
• Assess reported adult and family medicine symptoms and determine next steps per protocol
• Communicate patient concerns to providers and relay care instructions
• Document calls, messages, and care coordination in the EHR
• Coordinate referrals, prior authorizations, and follow-up appointments
• Provide patient education via phone or portal regarding medications, chronic conditions, preventive care, and warning signs
• Other responsibilities as assigned
Qualifications
• Active RN License in the state of TX
• 2+ years of primary care experience
• Associates or Bachelors Degree in Nursing
