Premise offers a wide range of dynamic, purpose-driven career opportunities. We are currently looking for a Full-time Nurse Practitioner (NP) to join our Digital Wellness Center team
Work schedule of 10-hour shifts (4x10) may vary within the following hours: 7am-7pm CST, Monday-Sunday. Schedule will include weekends and holidays and may be subject to change based on business need. Multiple state licensure will be required (up to 25 states). Must have reliable internet and be able to see ages 2+.
Essential Functions:
• Provides quality evidence-based medical care that is routinely provided in a primary care setting.
• Provide medical consultations including the diagnosing and treatment via the phone or video for Premise Health's patient population.
• Provides referrals both internally and externally to appropriate medical resource or specialty.
• Identifies emergency situations and provides care within legal scope of practice and as defined by Premise Health privileging process.
• Maintains medical records using EMR.
• May require other duties as assigned.
Job Requirements:
• Master's degree (MSN) required.
• Licensed as a Registered Nurse and Nurse Practitioner in state of practice.
• Nationally Certified as a Family or Adult Nurse Practitioner.
• Current Certification in AHA or ARC Basic Life Support for medical provider is required. Advanced Cardiac Life Support may also be required based on contract scope of services.
• Appropriate certification to write prescriptions under the authority of the Medical Leader, if allowed within state of practice.
• At least three years of post-residency clinical experience.
• Experience in a primary care, urgent care or emergency room setting. (Occupational healthcare experience may apply in some clinics).
• Prior telehealth health experience required.

While a number of factors influence salary, our estimated California, Colorado and Washington compensation is $+8,000- 130,000.
