Job Description
The Case Manager position will provide crisis triage, case management, and rehabilitation services or activities to our patients and:
Provide intensive case management/care coordination/advocacy services to promote client stability within the community
Perform all duties of the position, collaboratively with clinical, medical and administrative teams
Have the ability to compassionately engage, interact, and communicate with persons experiencing mental illness, emotional disturbance or chemical dependence in a positive and pro-social manner while maintaining appropriate personal and professional boundaries.
Actively coordinate with other community agencies and systems.
Other duties as assigned.
Bachelors Degree Required
Must have a valid Washington State Drivers License
Job Type: Full-time
Pay: $25.47 - $34.02 per hour
Expected hours: 40 per week
