Service: Hospital Operations - APU / ED

Contract: Fixed term, full time 1.0 FTE (80 hours per fortnight), Service operating Mon-Fri 8am to 4.30pm

About the Role

This new role aims to maximise timely multi-disciplinary decision-making for frail older people, 60 years plus, Maori, Pacific people, and others over 70 years, presenting to ED, supporting patient flow, quality of care and safe disposition.

You will join our Acute Frailty team working as an essential member of the ED in Hutt Hospital. The Geriatric Emergency Department Initiative (GEDI) role is a nurse led, physician championed model of care embedded within the Emergency Department.

The GEDI does not take over care of the older persons who present to ED. The GEDI nurse will assess which people, in the older cohort, would benefit from additional care co-ordination, fast tracking and focussing of clinical decision making and advocacy related to treatment and disposition.

This is a unique and diverse team that works with an interdisciplinary approach. The team works mainly in ED, but also across MAPU and Medical wards as required to follow through with short stay patient plans. The focus is to provide patient centred care, by the right discipline, at the right time and in the most appropriate location. We believe that the right support at the right time facilitates and support patient's recovery in their own home.

About the Team/Service Location

The Acute Health of Older People presence in the ED has been limited, with service provision from other Allied Health disciplines being provided by profession-specific inpatient teams, and by AWHI (Advancing Wellness at Home Initiative).

The setup of this new ED based role across the winter period is to support the ongoing development of the acute response in ED and support to the Shorter Stays in ED targets.

About you

You will need to have:

Essential:

Current New Zealand Annual Practising Certificate registration
Expert level in Professional Development Recognition Programme (PDRP) and committed to maintain Senior PDRP
Clinical Postgraduate Diploma is the required qualification.
A commitment to biculturalism.
An understanding of healthcare in New Zealand.
Desired:

5 years' experience working in the hospital and specialist services
Knowledge of standardised assessment and outcome measures.
Knowledge of ACC and MOH health funding streams.
Prioritise using time management and delegation skills.
Demonstrate ability to facilitate team working.
You will be able to: -

Work as effective team member.
Be comfortable to work in and develop in an interprofessional model
Show evidence of ongoing learning.
Demonstrate an understanding of significance of and obligations under Te Tiriti o Waitangi, including how to apply Te Tiriti principles in a meaningful way to your role.
