The opportunity
We have exciting opportunities for experienced Home Care Package Coordinators to join our Aged Community Care Team on a permanent, fulltime basis at Park Holme (we work out of Tregenza and Park Holme). This position offers the flexibility of a hybrid work arrangement, allowing you to work from home and our offices.
You’ll work closely with clients, carers, and families in the community to deliver person-centred care that supports independence and wellbeing.
Key Responsibilities
Provide person-centred case management and coordinate services to support clients and carers, including complex, high and moderate care
Conduct strengths-based assessments and develop tailored care plans aligned with client needs and contractual obligations
Manage HCP budgets and ensure resources are used effectively
Liaise with health professionals, community organisations, and internal teams to ensure holistic and coordinated client care delivery
Lead and support direct care staff
Ensure safe work practices through workplace assessments, hazard identification, and follow OSH guidelines
Maintain accurate records and meet reporting requirements
Navigate challenges such as limited resources, evolving technology, and complex client behaviours with resilience and adaptability
About you
You will have a strong background within the Aged Care Industry and have recent demonstrated HCP experience and/or strong knowledge, in addition you will be able to demonstrate the following experience, skills and attributes:
Certificate IV in Community Services (or equivalent) and a current First Aid
Strong experience in case management, care planning, and HCP budgets
Knowledge of service provision, coordination, allocation and compliance within a community environment
Strong decision-making, problem-solving, and time management skills
Able to work independently and as part of a team
Excellent communication and interpersonal skills
Confident using computer systems (Word, Excel, calendars)
