The Role
At Country Health Connect, Riverland Mallee Coorong Local Health Network (RMCLHN), we're redefining aged care, disability, and home support with a dynamic Person-Centred Care model. This approach empowers consumers to take charge of their lives, giving them the freedom to choose the services they need and the professionals who deliver them. As a Home Care Package Coordinator, you will be an integral part of a multi-disciplinary team, dedicated to developing, implementing, coordinating, and evaluating community-based programs. Your role will support individuals who wish to continue living comfortably in their own homes with the right assistance, making a meaningful impact in their lives.

This is a part time vacancy, working 60 hours per fortnight and salary will be adjusted according to actual hours worked.
Appointment will be subject to a satisfactory Criminal History Check and appropriate immunisation requirements.
