Limestone Coast Local Health Network, Country Health Connect - Home Support, Naracoorte
Salary: $67,58+ - $72,033 (pro rata) - OPS3
Ongoing Part Time (30 hours per fortnight 0.43FTE)
Working closely with the Home Support Coordinator and using a Primary Health Care approach, you will organise, provide and evaluate local services that meet the care and support needs of frail, aged and young disabled members in the community. Maintaining a client-centred approach to service provision, you will assist with planning and arranging specific Home Support services for individual clients to meet their needs. You will undertake Home Support assessments and regular reviews of local clients, develop care plans and set goals for clients. This will involve consulting with clients and their families to understand their needs and the kinds of services that will best meet those needs. As part of your role, you will direct, support and works closely with casual care workers, respite workers and aged care workers and contractors.
About You
You will have prior experience in working with older people and people with disabilities
Familiar with the administrative systems and duties required to support Country Health Connect clients. This will include experience in applying and maintaining strict eligibility criteria and service guidelines
You will have current knowledge of local and regional services, including aged care, community care and other programs relevant to the support of frail aged and young disabled community members.
Demonstrated knowledge of the impacts of ageing and disability on independence, wellness and health
Using your initiative and judgement when dealing with staff and clients, you will be capable of effectively liaising with people regarding sensitive issues.
Along with excellent communication and interpersonal skills, you will bring the ability to work in a self-directed manner within broad guidelines in a service provision setting.
