About the role:
As a Support Coordinator, you will play a key role in ensuring our clients receive high-quality, tailored services that meet their individual needs. You will act as the main point of contact for service users, families, and support staff, ensuring smooth coordination and effective communication.
Key Responsibilities
Develop and review person-centred support plans
Coordinate care and support services to meet individual needs
Liaise with families, healthcare professionals, and external agencies
Monitor service delivery and ensure compliance with policies and regulations
Provide guidance and supervision to support staff
Maintain accurate records and documentation
Respond to concerns, incidents, and service improvements proactively
Requirements
Previous experience in a care support, coordination role
Strong understanding of safeguarding and person-centred care
Excellent communication and organisational skills
Ability to manage multiple priorities effectively
Knowledge of the NDIS framework and an understanding of NDIS planning processes.
Leadership and problem-solving abilities
Relevant qualification ( in NDIS/ social/ community service preferred)
