At Joyous Ventures, we are committed to providing immediate support to individuals and families experiencing homelessness or crisis. Through our dedicated Social Services Team, we help participants navigate available resources and access comprehensive care.
We assist with:
Securing healthcare services (medical home, dental care, wellness programs)
Linkages to mental health and substance abuse support
Guidance with benefits enrollment (health insurance, income support)
On-site counseling, therapy, support groups, and assistance with emotional well-being and relationship challenges
Life skills development to foster independence
Community reintegration through social activities and connections
Transportation and accompaniment services
Additional support to achieve long-term housing stability
These services are crucial in promoting the well-being, empowerment, and resilience of our participants—strengthening the broader community in the process.
Job Summary
Participant Engagement & Intake
Coordinate intake, enrollment, and initial assessments for individuals with substance use histories, including justice-involved participants
Conduct biopsychosocial evaluations to identify needs related to housing, mental health, substance use, medical care, and social supports
Establish rapport using trauma-informed, culturally responsive, and nonjudgmental approaches
Care Planning & Service Coordination
Develop and maintain individualized service plans (ISPs) with SMART goals focused on recovery, stability, and reintegration
Connect participants to behavioral health services, detox and residential treatment, MAT (Medication-Assisted Treatment), peer support, and wraparound services
Facilitate referrals to housing programs, food assistance, transportation, and income support resources
Accompany participants to critical appointments and advocate for equitable access to care, especially in stigmatized systems
Clinical Support & Counseling
Utilize harm reduction principles and motivational interviewing to support participant autonomy and reduce barriers to engagement
Provide psychoeducation on substance use, relapse prevention, and coping strategies
Support treatment adherence, including medication management and coordination with prescribing providers
Facilitate weekly life skills groups focused on recovery, emotional regulation, and independent living
Lead house meetings to promote community norms, peer accountability, and shared decision-making
Documentation & Compliance
Maintain timely and accurate documentation of case notes, assessments, referrals, and care plans in funder databases (e.g., CHAMP, HMIS, CMS)
Track participant progress and prepare data for funder reporting and internal evaluation
Conduct regular chart audits to ensure compliance with DHS, JCOD, ODR, and Joyous Ventures standards
Collaboration & Systems Navigation
Participate in weekly case conferences with DHS, clinical teams, and external providers to coordinate care and address barriers
Liaise with internal departments and community partners to develop strength-based strategies for housing retention and wellness
Advocate for participants within healthcare, legal, and social service systems to ensure continuity of care
Crisis Response & Support
Provide 24/7 on-call crisis intervention, de-escalation, and emotional support as needed
Respond to behavioral health emergencies with calm, clinical judgment and appropriate escalation protocols
Maintain strict confidentiality and uphold HIPAA and agency policies at all times
