Competitive Salary: Band 5 - $83,038.80 + 12% superannuation
Location: Flexible working (mix of working from home and our Council Office in Sandringham)

About the Role:

Bayside City Council has an opportunity for an experienced non-clinical care partner to support our expanding Support at Home Program.

The non-clinical Care Partner is responsible for coordinating care for Support at Home clients to assist them to live at home as safely and independently as practicable. We take pride in setting our care partners up for success with manageable caseloads that support positive working relationships with clients that enable a focus on reliable and consistent care. You will join an aged care team committed and acknowledged for achieving exceptional outcomes for clients.

Key Selection Criteria:

To be successful in this role, you will bring the following skills and competencies:

A qualification in a health, allied health, social science or a community services discipline is desirable.
Experience in care management/care coordination.
Demonstrated understanding of the needs and challenges facing people who are ageing and wishing to remain at home within their community.
Experience in delivery of person-centred care and holistic care management.
An ability to identify client needs and to refer clinical issues while responding appropriately in an empathetic, objective, respectful and professional manner.
Well-developed written, verbal, conflict and problem resolution skills.
Mandatory pre-employment requirements:

Be willing to undergo and complete a satisfactory criminal history check.
Provide or obtain an employment-based Working with Children check.
Victorian driver’s licence.
