• You'll work closely with NDIS participants to achieve their goals, ensuring they receive the right supports to thrive.

• Connect participants with appropriate supports, services, and resources.
• Assist participants in understanding and implementing their NDIS plan.
• Maintain strong communication with participants, families, and other stakeholders.
• Accurately record and report on support coordination activities.
• Represent the best interests of NDIS participants, ensuring their needs and preferences are met.
• We are seeking a part-time employee for two days a week. Depending on performance and organisation needs, there is an opportunity for increasing in work days.
• Required Skills and Experience:
• Proven experience in support coordination or a related field.
• Strong communication and interpersonal skills.
• Excellent organizational and problem-solving abilities.
• Ability to work independently and as part of a team.
• Passion for working with people with disabilities.
• Relevant IT skills to ensure adequate skill in record keeping, report writing, NDIS portal access and information interpretation.
• This role is based in Chatswood, NSW.
• Some travel to client locations may be required.
