Responsibilities
Provide supportive pre-assessment engagement activities, through phone calls or in person, including problem-solving, patient education, and referral to resources
Deliver regular, individualized personal assistance to clients who are transitioning levels of care (e
, from detox to outpatient treatment or from residential treatment to outpatient) when most at risk for disengagement
Obtain written releases of information from the client to allow open communication between CWCC and referring entities, treatment agencies, family members, courts, and other agencies and individuals involved in the person's treatment and recovery support
Accurately and completely document all patient activity in complete notes prepared by close of business on the day of contact and ensure client files are maintained in accordance with CWCC's policies and procedures
Facilitate engagement groups for clients who are waiting to get into MH/SUD groups, provide crisis intervention and assessment to clients who present to the agency in coordination with a licensed mental health supervisor to assist with level of care determination
Attend community meetings with partnering agencies regarding procedures for accessing services specifically related to emergency situations
Liaison with referral agencies and maintain working relationships with local providers, community agencies, and state hospitals
Perform various administrative tasks, such as drafting correspondence, preparing reports, and managing office logistics
Attend other meetings as notified
Responsible for completing competency-based workplace safety training as assigned by the agency
