Reporting to a program manager, The Geriatric Nurse Case Manager will be a member of a multi-disciplinary team focused on serving seniors with geriatric related issues. He/she will provide all nursing case management functions for clients presenting with complex physical and mental health concerns. The responsibilities of this position include, but are not limited to:
Treats clients by using correct nursing techniques that are in compliance with all legislated requirements, the practice and ethical standards of the College of Nurses of Ontario and specific Reconnect procedures and policies;
Assists in the development and maintenance of effective linkages with hospitals, long term care and community service providers;
Identifies and communicates significant abnormalities in the client data to the appropriate team members;
Monitors the accuracy and timely completion of laboratory work;
Provides program teams, clients, families and others with specialized consultation / education related to nursing issues;
Develops, implements and documents treatment plans in conjunction with the client and other program team members;
Maintains client charts and other related client and workload measurement documents according to established policies and procedures;
Develops clinically sound case plans using the Ontario Common Assessment of Need (OCAN) or the Inter-RAI Community Health Assessment tool that are specific to the needs of clients and the objectives of the program in question;
Provides counselling to assist clients in accomplishing their goals and managing their symptoms and day-to-day stressors;
Consults with other service providers to promote continuity of care for clients;
Dispenses, administers and monitors medications, and keeps required records.
The successful candidate will possess:
Degree/or diploma from an accredited school of nursing, providing eligibility for a current Certificate of Competence as a Registered Nurse from the College of Nurses of Ontario;
Minimum 3 years’ experience in nursing; community geriatric nursing preferred;
Experience with injections, oxygen, catheter, IV, ostomy support and medication management required;
Psychiatric Nursing is a strong asset.
Key skills and abilities we are looking for:
Strong knowledge of the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care;
A well-defined sense of diplomacy, including solid negotiation, conflict resolution, and people management skills;
Experienced in working effectively as a member of a team, which includes internal and external stakeholders;
Excellent verbal and written communications skills;
Applied computer skills (Microsoft Office, Client Information Systems, Web browsing);
Regular access to a dependable vehicle and a valid Ontario driver's license in good standing is preferred;
Fluency in a second language is a strong asset.
Special Requirements:
Criminal background check
Must be willing to work flexible hours including weekends, evenings and statutory holidays as required.
Must be willing to provide services in the homes of clients as well as in residential care setting
