Job Description
About the Role
This is a full-time, work-from-home position with a US shift schedule (7:30 PM or 4:30 AM IST). The successful candidate will be responsible for coordinating patient care, providing education on disease management, and verifying patient demographics and medical information.
Key Responsibilities
• Manage a caseload of patients, providing daily monitoring and support via phone calls.
• Review patient-recorded vital signs, educate patients on improving diagnosed diseases, and assist them in following their care plan.
• Verify and record patient demographics, medical issues, medications, and risk management factors using specialized medical software.
• Handle potentially difficult conversations calmly and professionally, participating in individual and group projects as directed by management.
Requirements
To be successful in this role, you will need:
• A strong background in healthcare, preferably with experience in patient care coordination.
• Excellent communication skills, with the ability to engage patients in conversation and motivate them to follow their care plan.
• Strong organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
• A positive attitude and a passion for delivering excellent patient care.
What We Offer
We offer a competitive salary, estimated to be around $35,000 - $45,000 per annum, depending on experience, plus benefits and opportunities for growth and development.
