Our company, Evolve Healthcare LLC is currently seeking for Certified Occupational Therapy Assistant to perform treatment evaluated by the Occupational Therapist in and around Chicago Northside areas and nearby suburbs

Salary:

$40.00 to $50.00 /visit

Experience:

at least 1 year experience in homehealth setting preferred

Requirements:

COTA license in the State of Illinois
CPR certificate (classroom)
Valid drivers license
Recent Physical exam and TB Test
Experience with Therapy Boss and Kinnser EMR an advantage
New graduates are welcome to apply!
Job Type: Contract

Pay: $40.00 - $50.00 per hour

Work Location: On the road
