Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including long-term care, assisted living, and acute care environments. As an LPN, you will work closely with patients, families, and healthcare professionals to ensure optimal health outcomes and support the overall well-being of those in your care.

Responsibilities

Administer medications and treatments as prescribed by physicians
Conduct patient assessments and monitor vital signs
Assist with daily living activities and personal care for patients
Maintain accurate medical records and documentation
Collaborate with healthcare teams to develop and implement patient care plans
Provide education and support to patients and their families regarding health management
Perform procedures such as IV infusion, catheterization, and wound care
Ensure compliance with HIPAA regulations and maintain patient confidentiality
Participate in discharge planning and case management for patients transitioning to different levels of care
Experience
Candidates should possess a diverse range of experiences that enhance their ability to provide exceptional nursing care. Preferred qualifications include:

Experience in critical care, emergency medicine, or hospital settings
Familiarity with medical office operations, long-term care facilities, or assisted living environments
Proficiency in using EMR/EHR systems such as Epic, Cerner, or Athenahealth
Knowledge of ICD-9/ICD-10 coding and medical terminology
Experience working with individuals with developmental disabilities or dementia-related conditions
Skills in acute pain management, geriatrics, pediatrics, or hospice care
Background in home health care or community-based nursing
The successful candidate will demonstrate strong clinical skills, excellent communication abilities, and a commitment to providing compassionate patient-centered care.
