As a Navigator, Hospital Acquired Complications (HAC) at SALHN, you'll be at the forefront of driving positive change and enhancing patient safety across our healthcare services. Your expertise will be instrumental in identifying, investigating, and mitigating hospital-acquired complications, ensuring our patients receive the highest standard of care.
About the role:
The HAC Navigator reports to the Director of Clinical Governance and works closely with the Director of Medical Services. While the primary focus is monitoring Hospital Acquired Complications (HAC) outcomes and driving related improvements, the role may also involve exploring and addressing other areas of clinical variation.
The HAC Navigator will collaborate closely with the Clinical Governance Team and the Continuous Improvement Team. This role requires a self-reliant, motivated individual who can manage their workload independently while fostering effective collaboration with key stakeholders and maintaining a strong focus on continuous improvement.
Primary Objectives:
Support Prevention of Hospital Acquired Complications (HACs): Actively support processes within the SALHN HAC Improvement Model aimed at preventing HACs.
Data Validation: Ensure the accuracy and validity of HAC-related data by establishing process for reviewing coded HACs to confirm they are justified and accurate.
Build Relationships: Develop strong collaborative relationships with coders and clinicians and communicate directly with clinical units as needed to facilitate continuous improvement.
Identify and Support Improvement Opportunities: Pinpoint areas for improvement within the HAC model and support initiatives to enhance outcomes.
Monitor HAC Rates: Assist Standard Committees and other expert groups in monitoring HAC rates, conduct aggregate reviews as necessary, and develop systems to monitor prevention structures.
About you:
The HAC navigator will make informed decisions on a daily basis, such as identifying potential issues with HAC data, determining the best approach for process improvements, and deciding how to effectively engage with stakeholders,
The role involves identifying and addressing challenges related to HAC prevention and data accuracy. The HAC navigator must proactively seek solutions, anticipate potential issues, and implement corrective actions without waiting for direction,
ASO8: An appropriate tertiary and/or postgraduate qualification in Health Management, Project Management, or a related discipline, and/or equivalent relevant work experience.
RN3: Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia, holding or eligible to hold a current practicing certificate, along with appropriate work experience.
AHP4: An Allied Health degree or equivalent qualification that entitles registration with the relevant professional body, or, for self-regulated professions, eligibility for membership with the professional association, plus relevant work experience.
