Full job description
Heroes Home Health seeks a LPN to support case management in the provision of Home Health care and services. Heroes Home Health is a Home Health Agency that has served the Chicagoland area for more than ten years. Our facility is conveniently located within proximity to all major highways in the West Loop of Chicago. Heroes Home Health is committed to excellence and is always looking for talented health care professionals to join our team!

We are looking for a motivated LPN or QA specialist or case management to work with a team of skilled professionals. Your responsibilities will include, but are not limited to the following:

Responsible for the timely completion and accuracy of all clinical documentation including OASIS, Plan of Care, Visit Notes, etc.

Coordinates the completion and reviews all client medical records regarding OASIS
Performs all SOC, REC, ROC, DC OASIS reviews on a timely basis
Assures compliance with OASIS time frames as required by law
Assist with orientation of new home health care clinicians regarding OASIS process
Performs discharge summaries / OASIS activities, as appropriate, in conjunction with field staff
Reports to Agency Supervisor
Requirements

Graduate of an approved educational program
Minimum of 1+ years of OASIS review experience at a Home Health Agency
Extensive knowledge of CMS Home Health regulations including current COPs
Experience with AXXESS (EMR)
Knowledge or experience of ICD
Experienced applicants please.

LPN preferred (RN may apply)
