QUALIFICATIONS
High School Diploma or equivalent
Social Service Designee Certification
Good written and verbal communication skills
Good interpersonal skills
Good administrative and organizational abilities
Adheres to professional appearance
Knowledge of Social Resources in the Community
RESPONSIBILITIES
Ensure the residents’ psychosocial and concrete needs are identified and met in accordance with
federal, state and company requirements.
Assess the psychosocial, mental and emotional needs of residents.
Timely and accurate completion of the MDS Assessment.
Assist with orienting residents to the center.
Assist with discharge planning of residents.
Implement and update resident care plan and social history.
Communicate needs and the plan of care to residents, families, responsible parties and appropriate staff.
Assist with the facilitation of the Advance Directive, as indicated.
Assist with Theft/Loss process/system.
Assist with Grievance protocol.
Participate in abuse investigation and follow-up when required.
Participate in meetings, in-services and training as required.
Assist with the coordination of resident room moves.
Participate in the facility’s Behavior Management program, as indicated.
Provide Trauma Informed Screenings, as per guidelines.
Arrance ancillary services that have been determined necessary to maintain the residents’ concrete needs.
Coordination and maintenance of the IDT invitation process to residents and resident representatives.
Meet with consultants, as indicated or required.
CONSUMER SERVICE
Presents professional image to consumers through dress, behavior and speech.
Adheres to company standards for resolving consumer concerns.
Promotes and observes resident rights per regulatory guidelines.
Organize and maintain a professional work setting.
